This program is individualized to each patient care management to control and monitor drug interactions including side effects, patient support and education.

Features of this program include but are not limited to:

• Special packaging of medications to assist with therapy compliance
• Mobile application refill medication reminder via text / email
• A dedicated Pharmacy Support Team assigned to each patient for telephone or face to face counsel
• Mid-therapy calls for select medications / disease states to address potential adverse effects or adherence issues
